Your First Visit
Your 60-minute appointment with Dr. Bensoussan will include:
- Discussion of your needs & goals
- Medical history review
- Screening for oral cancer
- Evaluation of the temporomandibular jaw joints
- Assessment of your esthetics and bite
- Examination of the health of the teeth, gums, and bone
- Personalized treatment plan
- Financial information and payment plans
What to Bring
- Dental insurance card
- List of current medications
- Referral form from referring dentist (if applicable)
- X-rays from previous dental offices
- Questions or concerns about your dental health
Insurance & Billing
We are a fee-for-service practice. This means we don’t let insurance companies dictate the quality of care we provide to our patients.
Payment is required at the time of your visit. Then, we bill your insurance provider on your behalf to request a reimbursement. Our staff handles this process so you don’t need to worry about paperwork or dealing with insurance companies. Feel free to give us a call at (602) 242-2256 and our Insurance Coordinator will outline your insurance plan benefits and answer any questions.
